How much does this service cost?
Your health plan covers the expense of this service, making it available to you at no charge.
This service is completely free—there is no cost to you.
Do I have to sign a contract?
You do not need to sign a contract or make any long-term commitment. We’re simply here to support your health.
What does this service offer me?
You’ll be supported by a team of healthcare professionals who work with you to ensure your medications are being taken safely and effectively.

What kinds of services does the PlanPlus team provide, and how often will I be contacted?
Depending on your health needs, here’s what you can expect:
• Nurses may call you regularly to check how you’re doing—especially if you’ve recently been in the hospital or need extra support with managing your health.
• Pharmacy technicians or medical assistants may contact you to review your medications and gather important information.
• Pharmacists review your medications for any problems, such as drug interactions or side effects, and may reach out to your doctor if anything serious is found.
• We can also help with refill reminders, doctor appointment scheduling, and coordinating with your pharmacy or health plan when needed.
What is “medication adherence” and why does it matter?
Medication adherence means taking your medications every day as prescribed, with little to no missed doses.
It’s important because:
• Medications work best when taken the way your doctor intended.
• Missing doses can keep your condition from improving—or cause it to get worse.
What can happen if I don’t take my medication the way the doctor prescribed?
