Security Health Plan Partners with PlanPlus by Curant Health to Reduce Total Cost of Care (TCoC)
PlanPlus and Security Health Plan of Wisconsin have partnered to deliver proven Medication Care Management services to members with Congestive Heart Failure, aiming to improve health outcomes, boost medication adherence, and reduce Total Cost of Care.
March 28, 2024 (Atlanta, GA) – Security Health Plan of Wisconsin, Inc. and PlanPlus by Curant Health (PlanPlus) have launched a strategic partnership aimed at achieving Total Cost of Care (TCoC) savings for Security Health Plan. Beginning on April 1, PlanPlus will provide its proven Medication Care Management (MCM®) services to eligible Security Health Plan members with Congestive Heart Failure (CHF) to improve the health of members and deliver a calculated positive return on investment while providing insights around total direct and indirect cost burden. Secondary objectives include increasing the proportion of members adherent to their CHF medications, improving member quality of life (QoL), and improving member satisfaction as measured by Net Promoter Score (NPS).
PlanPlus will empower Security Health Plan members through education, preventive care initiatives, and personalized health management tools to promote proactive health behaviors and remove adherence barriers, leading to improved medication adherence and reducing the need for costly interventions. This program adds to PlanPlus’s growing list of partnerships that have consistently produced validated outcomes over the last two decades including reduction in hospitalizations, reduced cost of care, and improved plan Star ratings.
